I have a friend who's existence is being the Avatar of Murphy's Law.
That sounds like the kind of thing that would happen to him…
He BSoD'd on a BSoD, tripped up stairs and then fell back down the stairs, and managed to start a grease fire in his cornflakes because of a freak accident involving a careless roommate with a cigarette and a gas leak in the cereal cupboard. And it ended up more or less looking like this:

So it's a square piece that erases a square shaped section of blocks and removes any points they were worth?
Or is it a 4x4 hollow square? (not likely, it's called tetris because each piece is 4 segments large)

In my experience, this only happens during knockoff Tetris games. In official products like the NES version or GB version the pieces all appear pretty evenly.
